| Telereferral and telecounselling for Haematology, Oncology and Nephrology |

Objectives
The main objectives of the service are the following:
- bringing specialist care closer to patients;
- improving communication between Primary and Secondary Care;
- enhancing the problem solving capability of the GP through his/her close collaboration with specialists. |

Description
A description of the current workflow has been provided in the proposal for this Project previously submitted to the Commission.
The Telereferral service is based on two main pillars:
- Shared Electronic Clinical Records;
- High-quality video-conferencing equipment.
The current flow will be modified as follows for a Virtual Referral request:
1. the request for a Virtual Referral is introduced in the diary of the specialist within a slot earmarked for Virtual Referral that each of the specialities defines;
2. the Primary Care physician receives confirmation of the appointment;
3. The Virtual Referral is carried out through video-conferencing;
4. The specialist fills in the report of the tele-consultation that will be stored in the patient?s Shared Clinical Records.
